The fatal incident occurred at night along the Bauchi–Dass Road in Yelwa, near the POWA shops by the Police Barracks. According to eyewitnesses, Lazarus was knocked down by a hit-and-run driver whose vehicle had no headlights and fled the scene toward the Rafin Zurfi axis of the Tafawa-Balewa Road.
